About the Role

  • As our full-time Case Manager, you'll work in a deeply rewarding and supportive environment, providing one-on-one support to adults in our supportive housing community. You'll help clients set goals, connect with resources, and maintain stable housing. You'll also screen individuals and families seeking affordable housing options and guide them through the process with compassion and care.
  • Assess individuals and families in housing readiness, needs, and strengths
  • Provide weekly case-management services to assigned clients in the field, including working with clients to determine client support plan, goals, resources, and maintaining client case notes, history, and data through HMIS (Unity), utilizing best practices for keeping client information confidential.
  • May attend CoC and other community meetings, as well as present Ibis programs to community stakeholders
  • Oversees data quality of all client files upon entry and exit from the program.
  • Coordinates psycho-educational groups.
  • Monitors monthly coordinated entry follow-ups and verifies all updates are made within the first week of each month.
  • Participate in staff and group meetings. May organize and run treatment and community meetings
  • Maintains client case notes in our EMR, history, and data, utilizing best practices to keep client information confidential. Generates and compiles client outcomes and reports within Clarity for reporting to funders for the program. Keep confidential records according to the Code of Ethics.
  • Maintain minimum weekly contact to track client progress.
  • Maintain professional boundaries.
  • May provide supervision to Interns (based on experience), ensuring that proper case management guidelines are being followed.
  • Visit and meet with community service providers, service organizations, and institutions that prove beneficial to the maintenance of self-sufficient living.
  • Meet personal goals and actively contribute to the achievement of team and organizational targets/goals. Attend required staff meetings and ongoing training. Complete other duties as assigned.

What You Bring

  • Bachelor's degree in social work* preferred, or Human Services related field
  • 2-3 years' experience in case management or human services
  • Understanding of housing systems, community resources, and behavioral health is a plus
  • Bilingual (Spanish/English) is a plus
  • Empathy, organization, and a heart for service
  • Must be at least 21 years of age, possess a valid Florida driver's license and insurance, and have reliable personal transportation
  • Must have excellent interpersonal skills
